Our Assisted Living Intake Process

At 1 on 1 Comprehensive Healthcare Solution, we strive to make the transition into assisted living as smooth and supportive as possible. Our intake process is designed to ensure that every individual receives personalized care in a safe, comfortable environment. Below is an overview of the steps involved:

1. Initial Inquiry

Families, case managers, or prospective residents may contact us via phone, email, or our online form to express interest in placement. Our intake team will provide general information about our services, community locations, and availability.

2. Application Submission

To begin the formal intake process, an application for placement must be completed. This can be submitted by the applicant, a family member, or a referring agency.

3. Review of Information

Our team will review the submitted application along with any relevant medical, psychological, or social assessments to determine whether we can appropriately meet the applicant’s needs.

4. Assessment and Consultation

If needed, we will coordinate a consultation with the applicant, their representative, and healthcare providers to clarify care requirements and ensure compatibility with our services.

5. Acceptance and Placement

Once a suitable match is confirmed, we will offer placement at one of our residential locations. Our team will provide details on the home, room availability, care plan, and move-in preparation.

6. Move-In and Orientation

Upon admission, we guide the resident and their family through the onboarding process, including a tour, staff introductions, and review of the personalized care plan. We aim to make every new resident feel at home from day one.
