- Facility Ran and overseen by a Nurse Practitioner with Doctoral Degree
- Weekly outing and daily social activities
- Facility owned Handicap accessible Van.
- Resident and their families fully involved in all decision making.
- Physician Services on-site (Visiting Physician/Nurse Practitioner)
- Physical and Occupational Therapy (May be arranged in home or out of facility)
- Help with community integration or re-integration.
- 24/7 Staff Support
- Well Trained staff
How to Qualify for our Group Home Services?
To qualify for our services, individuals must meet the following criteria:
- Age: 18 years or older.
- Diagnosis: Documented mental or physical illness, brain injury, or other disabilities.
- Care Plan Compliance: Willingness to follow care plans developed by medical providers.
- Funding Options: Availability of funding through programs such as the Community Alternatives for Disabled Individuals (CADI) waiver, Brain Injury (BI) waiver, Elderly Waiver (EW), Group Residential Housing (GRH) program, Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), or Retirement, Survivors, and Disability Insurance (RSDI). Private Pay is also welcomed.
