Job Summary

The Registered Nurse (RN) plays a vital role in ensuring the health, safety, and well-being of our residents. The RN is responsible for assessing patient needs, developing and implementing care plans, administering medications, and collaborating with healthcare providers to deliver optimal care.

Key Responsibilities

  • Conduct comprehensive assessments and develop individualized care plans for residents.
  • Administer medications and treatments according to physician orders and facility policies.
  • Monitor residents’ health status, identify changes in condition, and take appropriate action.
  • Provide wound care, trach care, and other specialized nursing interventions as needed.
  • Educate residents and caregivers on disease management, medication adherence, and wellness strategies.
  • Maintain accurate and up-to-date medical records, documentation, and reports.
  • Coordinate and communicate with physicians, therapists, and other healthcare professionals to ensure continuity of care.
  • Assist with mobility, transfers, and daily living activities as needed.
  • Advocate for residents’ healthcare needs and ensure compliance with all regulations and safety standards.
  • Provide leadership, training, and guidance to direct care staff.

Qualifications

  • Valid Registered Nurse (RN) license in the state of Minnesota.
  • Minimum of one year of nursing experience, preferably in assisted living, long-term care, or a group home setting.
  • Strong clinical assessment and critical thinking skills.
  • Knowledge of medication administration, wound care, and chronic disease management.
  • Ability to work independently and collaboratively in a team-oriented environment.
  • Excellent communication, documentation, and organizational skills.
  • CPR and First Aid certification (or willingness to obtain).
  • Familiarity with state and federal healthcare regulations.
  • Assisted Director License or willing to obtain.  

Preferred Qualifications

  • Experience working with individuals with mental health conditions, brain injuries, or physical disabilities.
  • Knowledge of waiver programs (CADI, BI, EW) and group home regulations.
  • Experience with electronic medical records (EMR) systems.
